Use the form below to change your Health Catalyst account password. If you do not know your Health Catalyst Password, use the Forgot Health Catalyst Password utility to reset it.

Note: The password chosen should be a minimum of 12 characters and should meet all of the criteria below.
       - Password includes at least one lower-case, alphabetic character (e.g. a-z)
       - Password includes at least one upper-case, alphabetic character (e.g. A-Z)
       - Password includes at least one numeric character (e.g. 0-9)
       - Password includes at least one special character from this list( $, # , @ ).
